宛名 refers to the name of the person the letter is addressed to, and their address.

From a questionaire 葉書（はがき）, a post card.

恐れ入りますが５０円切手をお貼りください
おそれいりますが５０（ごじゅう）えんきってをおはりください

[Awfully sorry, but] please affix a 50 yen stamp.

恐れ入りますが…　is a phrase used when asking someone of a higher standing (like customers) than you to do something. It's similar to お手数ですが which came up in this post.
